Polk County Medical Society Receives HCAP GRANT

The Polk County Medical Society has received a grant from the Health Access Partnership, Healthy Community Access Program (HCAP). The grant was awarded to address access to health care for the uninsured in Polk County.

The grant, which is offered through the U. S. Department of Health and Human Services, was awarded to the Health Access Partnership, which is a volunteer coalition of community agencies and individuals aligned with the Healthy Polk 2010 movement.

The Polk County Medical Society part of the grant will focus on seeking physician volunteers to provide care to the uninsured. A patient will be referred on to a specialist who has signed up for care through the Polk County Medical Society on a rotating basis.

The benefits of this program for physicians to participate in is that all patients are screened for eligibility of assistance prior to presenting to your office, the physician will receive referrals on a rotating basis, and physicians will be recognized for their contributions.

Juliet R. Bascom has joined the Polk County Medical Society as the Volunteer Physician Coordinator for the CAP Grant program. Juliet comes to us with years of experience working with physicians.
